Herein, we report our successful experience with intercostal artery perforator (ICAP) flaps for correcting trunk defects, without any significant complications arising. During the period spanning from March 2015 to March 2019, fourteen patients experienced reconstruction using ICAP flaps. Patient data, including age, sex, the reason for the defect, its size, the perforator’s location, flap size, any complications, and the duration of follow-up, were subjected to a retrospective review. A striking average age of 565 years was identified among the patients, with the age range extending from a low of 19 to a high of 80 years. Given the bacterial culture results from the wound, which indicated no microbial growth, all operations were conducted. Doppler ultrasonography revealed the presence of reliable perforators surrounding the defect. With the assistance of a pulsatile perforator, the perforator flaps were elevated and rotated in a propeller-like fashion, aligning with the defects. Our surgical procedure involved the placement of five dorsal and two lateral ICAP flaps. The average size of the flaps was 1255 square centimeters (ranging from 65 to 188 square centimeters). A primary closure of the donor site was carried out. Amongst the observed complications, marginal congestion emerged in one case, but it resolved completely, eliminating the need for revisionary surgery. Eight months constituted the mean follow-up time in this study. Every patient reported satisfaction with the surgical results. Mobilizing ICAP flaps effortlessly decreases donor site morbidity, maintaining the integrity of the underlying muscles essential for trunk reconstruction. Thus, these flaps constitute advantageous options in the rebuilding of trunk defects.
Hypothenar free flaps (HTFFs) serve as a widely practiced method for repairing defects in the palm. Although anatomical and clinical research on HTFF has been pursued, this method remains subject to certain constraints. This research provides guidelines for the design of expansive flaps, maximizing the ease of HTFF harvest and minimizing donor site morbidity. In the area of hand defect reconstruction, a count of 14 HTFF procedures was made. A relaxed skin tension line, originating from the fourth web space and extending to a point 10 mm ulnar of the pisiform, served as the directional principle for the oblique flap’s design within the proximal HT area. The ulnar digital artery and the basilic vein’s HT branch are connected to the flap pedicle via one or two perforators. Moreover, a branch of the ulnar digital nerve allows for the harvesting of innervated HTFF. By examining the electronic medical records, details regarding patient attributes, operative information, and follow-up duration were collected. Furthermore, the patient’s surgical outcome score, assessed on a scale of 1 to 10 points, was documented at the final follow-up visit. The mean harvest duration was 46 minutes; two perforators were incorporated in 10 cases. In the analysis, the mean flap area was established as 1084 square centimeters. At the donor sites, there were no issues like donor site depression, scar contracture, keloids, wound dehiscence, numbness, neuroma pain, or hypersensitivity. Similarly, the flap sites exhibited no functional or aesthetic problems like cold intolerance. The surgical repair of palmar defects proves to be a complex challenge for hand surgeons. Nevertheless, substantial HTFF yields are achievable without procedural difficulties through the oblique axis HTFF method. We are certain that our surgical tips raise the effectiveness of HTFF for the reconstruction of palmar defects.
An untreated extremity’s lymphedema results in a rise in volume. Management of this condition may include conservative or surgical therapies. jnj-26481585 inhibitor Lymphological liposculpture, a two-part process involving resection and conservative monitoring, is used to achieve curative volume adjustment in the extremities of patients with secondary lymphedema. A significant decrease in the use of complex decongestive therapy (CDT) is achieved via this treatment. Between 2005 and 2020, our practice and clinic treated 3184 patients who developed secondary lymphedema following breast cancer or gynecological malignancies. In a study involving 65 patients who underwent lymphological liposculpture, perometry and questionnaires were used to record and assess data. Aligning the sick side with the healthy side was accomplished in every single patient. CDT treatment was completely ceased postoperatively in 5842% (n=38) of patients; concurrently, a lasting reduction in CDT was accomplished in 3382% (n=22). Despite the procedure, the postoperative CDT in 769% of patients (n=5) could not be diminished. Ninety-two point three percent (n=60) of the patients indicated a sustained and meaningful improvement in the quality of their lives. Secondary lymphedema finds a sustainable, curative solution in lymphological liposculpture, a standardized procedure that addresses extremity volume and postoperative CDT, resulting in improved quality of life for patients.
Cranioplasty benefiting from metal fixation systems for drill-created cranial bone flaps often experience numerous complications. A fine diamond-coated threadwire saw (diamond T-saw) is utilized in a modified craniotomy procedure to reduce the bony defect, complemented by osteoplasty using calcium phosphate cement without metallic fixation. We outline the results and guidance associated with this technique. From 2015 to 2019, 78 consecutive patients underwent elective procedures for clipping unruptured intracranial aneurysms via frontotemporal craniotomy. Follow-up periods extended from 13 months to a maximum of 66 months. Bone computed tomography (CT) and 3D computed tomography (3D-CT) were employed to evaluate the bone fixation status. By using the diamond T-saw, the bone defect could be significantly lessened. Post-surgery, there was only one occurrence of wound infection within the first week, with no further infections being reported. No pain, no visible shifts of the bone flap, no fluid pockets, and no other complications were identified. Bone CT and 3D-CT scans confirmed the presence of good bone fusion in all patients, coupled with thoroughly satisfactory outcomes for bone fixation and cosmetic appearance. No complications, characteristic of metal fixation procedures, were observed. Employing a method that is technically straightforward and safe, we achieved satisfactory mid-term bone flap fixation, hence its potential to facilitate bone fixation without resorting to metal plates.
In the modern era’s embrace of scientific medicine, some people maintain a preference for folk remedies as an alternative healthcare modality. Nevertheless, the erroneous application or use of traditional remedies can lead to harmful consequences. This study investigated the clinical aspects of burns arising from folk remedy use among the potential complications. Clinical records of patients who sustained burns from folk remedies between May 2015 and April 2022 were examined in a retrospective study. The study gathered data on patient attributes such as age and gender, the specifics of the folk remedy, the purpose of its application, burn severity, the number of wounds, lesion type, and the treatment approach. Fifty-nine patients presented with burns attributed to the utilization of folk remedies in our study. Among the subjects, females accounted for 763%, and their mean age was 60 years, representing 729% of the total. Glacial acetic acid, while a notable folk remedy (203%), was surpassed in frequency by moxibustion (746%). Key reasons for the use of folk remedies were the mitigation of arthralgia (39%), the pursuit of improved health (186%), and the treatment of tinea pedis (119%). The majority of patients experienced multiple wound sites and severe burns requiring surgical intervention; this accounted for 729% of the cases. Lower extremity lesions, including those on the foot, constituted a significant proportion. Folk remedies, the subject of this study, were found to present a risk of burns, with the study also detailing the wounds’ clinical features. The findings underscore a need for increased public understanding of burn hazards associated with traditional remedies.
The first free abdominoplasty flap for breast reconstruction, as detailed by Holmstrom, was introduced over four decades ago. Surgical improvements and technological innovations have contributed to the broad acceptance of autologous breast reconstruction, re-establishing the feminine breast following mastectomy procedures. While concepts and techniques are in a state of constant advancement, keeping a bird’s-eye view of the situation is proving difficult. This article provides a review, encompassing current trends and recent innovations, related to autologous breast reconstruction.
School-age children and adolescents are frequently affected by ingrown toenails, although this condition can occur at any point in a person’s life. The causes of ingrown toenails frequently involve a complex interplay of increased nail curvature, trauma, and the force of external pressure. The spectrum of ingrown toenail treatment includes both conservative and surgical procedures. Conservative treatment can be carried out by using different approaches, including gutter splints, dental floss, and cotton. A bifurcated approach to surgical nail procedures typically involves decreasing the size of the nail plate and removing surrounding tissue masses. Although various conservative and surgical approaches were attempted, a high recurrence rate remained a significant challenge. Therefore, a permanent surgical technique, utilizing a paronychium flap, was utilized to address the issue of excessive periungual tissue and curved ingrown toenails in a 15-year-old male adolescent who had not responded favorably to conservative and multiple surgical treatments administered over four years. The toenail’s form persisted consistently for 22 months after the surgery, and no reported instances of walking-related pain or inflammation were noted. For advanced ingrown toenails, characterized by an abundance of periungual tissue and nail curvature, this surgical procedure is a possible solution, anticipated to provide lasting relief and permanent effects.
